Requesting a copy of your medical records

Emerald Document Imaging is now the medical records custodian for PL Pediatrics. SA Medical no longer provides copies of records from the practice.

Request records online

Authorized requestors can submit a request through Emerald’s online medical records release form.

What happens next

  1. Fill out the online release form on Emerald’s website.
  2. Pay Emerald’s fee.
  3. Once payment is received, Emerald emails you a secure link to download the chart.

Fees

Copy of chart, by secure download$25.00
Mailed CD, if requested+ $10.00

Emerald releases only a copy of the full chart. Partial records cannot be requested.
